§1 Panorama General: Péptidos en Medicina de Longevidad¶
1.1 Definición y contexto¶
Los péptidos terapéuticos son cadenas cortas de aminoácidos (2–50 aa) con actividad biológica específica. En la última década han pasado de la investigación básica a las clínicas de longevidad, generando un mercado global estimado en >10.000 M USD (2025).
Problema central: la brecha entre el entusiasmo clínico y la evidencia real es enorme. La mayoría de péptidos usados en longevidad tienen nivel de evidencia [C-D] en humanos, con datos predominantemente animales.

1.4 Actualización regulatoria 2026¶

- Reacciones adversas reportadas: reacción en punto de inyección (leve), eritema local
2.7 Recomendación clínica¶
No prescribir como primera línea para ninguna indicación. Evidencia insuficiente para justificar uso rutinario. Si el paciente lo solicita específicamente tras información completa, documentar consentimiento informado detallado indicando: (1) ausencia de RCTs, (2) procedencia de datos de un solo laboratorio, (3) incertidumbre sobre seguridad a largo plazo.

§3 Thymosin Alpha-1 (Zadaxin)¶

3.8 Recomendación clínica¶
El péptido con mejor relación evidencia/riesgo para longevidad. Justificable para pacientes con inmunosenescencia documentada (linfopenia, infecciones recurrentes, respuesta vacunal pobre). Monitorizar: hemograma completo con subpoblaciones linfocitarias cada 3 meses.

Datos más sólidos de los péptidos de segunda línea: - Modelo murino de infarto: reducción del tamaño del infarto en 40-50% [C] - Wound healing: aceleración significativa en modelos animales [B en veterinaria] - Uso veterinario extenso en caballos de competición (evidencia práctica pero no publicada como RCTs) - Sin RCTs en humanos publicados a fecha 2026
Combinación frecuente: TB-500 + BPC-157 ("stack de reparación"). Sin evidencia de sinergia en humanos. Racional teórico basado en mecanismos complementarios (angiogénesis + citoprotección).
4.3 Epithalon — Análisis crítico¶
La promesa: activación de telomerasa → extensión telomérica → longevidad.
La realidad: - Investigación del grupo Khavinson (San Petersburgo) — situación similar a BPC-157/Zagreb: un solo grupo - Publicaciones mayoritariamente en revistas rusas con peer review cuestionable - Estudios en humanos: solo series de casos y estudios observacionales rusos - Mecanismo plausible pero calidad de evidencia demasiado baja para recomendar - Ningún estudio ha medido telómeros antes/después con metodología rigurosa

4.5 Selank y Semax — El caso ruso¶
Ambos péptidos están aprobados en Rusia como medicamentos con indicaciones específicas: - Selank: trastorno de ansiedad generalizada (registro ruso) - Semax: ictus isquémico agudo, deterioro cognitivo (registro ruso)
Problema: los estándares regulatorios rusos no son equivalentes a EMA/FDA. Los estudios pivotales no están disponibles en inglés ni en bases de datos occidentales. La evidencia es difícil de evaluar según criterios GRADE.
§5 Agonistas GLP-1: Semaglutida y Tirzepatida¶

Los agonistas del receptor GLP-1 representan la revolución farmacológica más importante en medicina metabólica de la última década y, posiblemente, la primera clase de fármacos con evidencia sólida de efecto pleitrópico sobre longevidad [A].

5.8 Recomendación clínica¶
RECOMENDACIÓN FUERTE [A]: Los agonistas GLP-1 son la intervención farmacológica con mejor evidencia en longevidad metabólica y cardiovascular. Indicados en todo paciente con IMC ≥27 y riesgo CV, con o sin diabetes. Considerar dosis bajas off-label en pacientes con normopeso e inflamación crónica de bajo grado (PCR-hs elevada), aunque esta última indicación carece de evidencia directa.

§6 Secretagogos de GH: La Paradoja IGF-1 / Longevidad¶
6.1 La paradoja central¶
La evidencia más sólida en biología del envejecimiento indica que MENOS GH/IGF-1 = MAYOR longevidad:

NO justificado como "anti-aging" en individuos sanos.
6.6 Recomendación clínica¶
NO RECOMENDAR secretagogos de GH para longevidad. La evidencia apunta consistentemente a que elevar GH/IGF-1 ACORTA la vida. Si el paciente lo solicita, explicar la paradoja IGF-1 con evidencia. Si insiste, documentar: (1) consentimiento informado con riesgos, (2) screening oncológico previo obligatorio, (3) monitorización trimestral de IGF-1, glucosa, HbA1c, PSA (hombres).

NAD+ (nicotinamida adenina dinucleótido) es cofactor esencial en >500 reacciones enzimáticas. Declina ~50% entre los 40-60 años. Implicado en:

§9 EDTA y Quelación: Estado de la Evidencia Post-TACT2¶
9.1 Contexto histórico¶
La quelación con EDTA (ácido etilendiaminotetraacético) se propuso como tratamiento para enfermedad cardiovascular desde los años 1950, basándose en la hipótesis de que la eliminación de metales pesados (plomo, cadmio) y calcio reduciría la progresión aterosclerótica.

9.3 ¿Por qué TACT2 no replicó TACT1?¶
Hipótesis principal: los niveles de plomo poblacionales han disminuido significativamente desde la eliminación del plomo de la gasolina. En la era TACT1, los niveles medios eran ~35% más altos. Si el beneficio de TACT1 provenía de la quelación de plomo, la menor carga de plomo en TACT2 explica la ausencia de beneficio.
Implicación clínica: la quelación con EDTA puede ser inútil en poblaciones con niveles de plomo bajos actuales.

NO RECOMENDAR quelación EDTA de forma rutinaria para prevención CV. El fracaso de TACT2 en replicar los hallazgos de TACT1 debilita sustancialmente el caso. Única excepción potencial: pacientes con niveles de plomo documentadamente elevados (>25 µg/L) en los que la quelación tiene indicación toxicológica independiente [B].

§10 H₂O₂ IV: Terapia Prohibida¶
10.1 Declaración categórica¶
NUNCA OFRECER NI ADMINISTRAR PERÓXIDO DE HIDRÓGENO (H₂O₂) INTRAVENOSO.
10.2 Muertes documentadas¶
Se han reportado muertes por embolia gaseosa tras administración IV de H₂O₂. Caso documentado en la literatura: infusión IV mortal de peróxido de hidrógeno → embolia de oxígeno → muerte, con evidencia bioquímica de peroxidación lipídica cerebral y ataque por radical hidroxilo (Lubec B et al., Free Radic Biol Med 1996;21(2):219-223). PMID: 8818637 · https://doi.org/10.1016/0891-5849(96)00018-4 — el propio artículo advierte que «la muerte tras administración de peróxido en humanos se ha reportado repetidamente». El mecanismo es simple y letal:

NUNCA prescribir péptidos obtenidos por el paciente de fuentes no reguladas (internet, mercado gris). Si el paciente ya los usa, documentar y ofrecer alternativa de farmacia magistral autorizada.

S2. Riesgos específicos de la vía IV¶
- Extravasación: especial cuidado con Ca²⁺, vit C alta osmolaridad y ALA (irritante). Vigilar el punto de punción; detener ante dolor/edema.

Publicly states that it is a major pharmacy with core competence in formulación magistral for individualized treatments, with GMP-level quality claims, and that it only compounds medications under medical prescription.[3][4] It also hosts a public formula preparation / control request form (“hoja de encargo de elaboración y/o control de fórmulas magistrales o preparados oficiales”), which explicitly frames the workflow as pharmacy-to-pharmacy / prescriber-linked compounding.[5]
Operational implication: Xalabarder is a plausible Barcelona compounding partner for physician-specified individualized IV formulations, but not a public source for ready-made “longevity drip” formulas.

Non-negotiable operational rules pulled from public protocols¶
- NAD should not be mixed in the same infusion bag with vitamins/minerals/medications in the Hydreight protocol; it is reconstituted and placed into a 500 mL normal saline bag.[11]
- High-dose IV vitamin C is not an IV push protocol. Riordan specifies slow IV drip, generally 0.5 g/min (with up to 1.0 g/min requiring close observation), and explicitly says it should never be given as an IV push.[14]
- G6PD screening is mandatory before high-dose IV vitamin C in the Riordan protocol; baseline chemistry, CBC, and urinalysis are also recommended.[14]
- There is no standardized “longevity drip” formula. ACCP explicitly states there is little evidence for benefit in wellness hydration/vitamin infusions, no standardization of mixtures, and concludes there is little to no evidence that benefits outweigh the risks for routine hydration and vitamin infusions.[16]
- Compounding quality matters. ACCP recommends physician approval, licensed personnel, pharmaceutical-grade ingredients, sterile compounding, immediate availability of medical care, and informed consent.[16]
